In modern and asymmetric conflicts, traumatic airway obstruction caused by penetrating injury to the face and neck anatomy was the second leading cause of preventable mortality. In particular in case of mass casualties and according to downstream evacuation type, tactical context conditions the choice of the definitive airway technique. Definitive airway management in the emergency setting is most commonly accomplished by endotracheal intubation. When this one fail or is not possible, a surgical airway, usually cricothyrotomy, is indicated.

We report the case of a complex tactical indication of a primary cricothyrotomy in a severe cervicofacial injury with indication to definitive airway management, without possibility of sedation and mechanical ventilation during his evacuation.
